TITLE: POSE METHOD OF RUNNING FOR THE TREATMENT OF CHRONIC ACHILLES TENDINOSIS

runners who have the diagnosis of chronic Achilles tendinosis (degenerative changes of the Achilles tendon)

INTRODUCTION
Overuse injuries mainly localized in the lower extremities seem to be the price you have to pay being a runner.
Knee and shin injuries are more frequent than achilles tendon problems. But there are more serious consequences to be dealt with: Achilles tendon pathology leads earlier and much more often to a reduction of training volume or complete stop of running activity, to visits at the doctors office or to a complete end of the sporting career than any other injury.
The classification of Achilles tendon pathology differentiates insertional (insertional tendinitis, Haglund heel, retrocalcanear bursitis) from non-insertional problems (achilles tendinitis, achilles tendinosis).
The most important achilles tendinosis is a degenerative disorder, not an inflammation, with following clinical features:
- Pain localised 2-6 cm above the insertion of the Achilles tendon
- in the morning when getting up
- every time when starting weight bearing
- Stiffness when starting weight bearing after prolonged pause
- Slow reduction of pain with prolonged weightbearing and aggravation later on
- Localized pain on palpation
- Localized swelling of the tendon
Numerous risk factors are associated with the developement of Achilles tendinosis (training volume, training surface, shoes, biomechanical factors...), and it's management is based more on empirical than scientific principles.
In a recent study for treatment of degenerative Achilles tendinosis a specially designed eccentric calf muscle training program showed excellent results in all participants.
As Pose Method of Running is characterized by less eccentric work at the knee and more eccentric work at the ankle, this leads to the theoretical model, that Pose could have an important role in the treatment of Achilles tendinosis in runners.
In this study we evaluate use of the Pose Method of Running for the treatment of chronic Achilles tendinosis.
